FLD file definition available?

it appears that the Carlson FLD file has about 110 data columns. Only the first 20 have headers - and only those headers can be used in a code definition report.

Does anyone have a list of all the columns? I have found a few:
21 Distinct pt layer name
22 Symbol layer name (when using separate attribute layers)
51 point number layer (when using separate attribute layers)
52 point elevation layer (when using separate attribute layers)
53 point description layer (when using separate attribute layers)
57 elevation prefix
58 elevation suffix
59 point group name
