Survey 2008 Standalone Toolbar Icons

I just recently installed Survey 2008 Standalone onto my Dell Worskstation (2.8 ghz, 3 gig ram, sata drive) and the Icons for all toolbars do not show. The functionality is there, but the toolbars are blank.

I re-installed from the latest build with the same result. Any suggestions?

Type CUILOAD at the command line unload SURV then reload it by hitting the Browse button and select the SURV.CUI file. IF this does not bring up the icons then type CUI at the command line. This will open the Custom User Interface. Under the Customize tab - open the dbcon.cui and apply - once applied then reopen the SURV.cui this should reset all the icon images.
